One sheet must be filled in to properly identify your school and should be placed directly beneath the school’s filled-in Iowa Assessments™ Scoring Worksheet. For schools wanting separate scores for multiple campuses, multiple sheets are needed.

One sheet must be filled in for each grade and used as a cover sheet to divide the grades. If a school has more than one class in a grade, multiple Grade/Class ID Sheets are needed for that grade. This helps Testing & Evaluation properly identify each grade and each class within a grade for scoring.
